Apply to Adopt

Please fill out the below form to submit your adoption application. 

Please understand that your answers to these questions help us determine the best homes for our animals. By applying for the adoption of a FREEdom Tails Ohio Rescue, you hereby give your veterinarian(s) and other references permission to disclose pertinent information to FREEdom Tails Ohio. Adoption fees for our pets include spay/neuter, microchipping, and up to date vaccinations for the pet's age.

 

You must include the name(s) of the specific animal or animals you are interested in adopting.

Applications will not be processed until application form and household photos have been submitted.
Incomplete applications will not be processed.

 

Please be patient as we review your application. Due to the volume of applications FREEdom Tails receives on a daily basis, we are only able to contact those applicants being asked to take the next steps in the application process. We apologize for any inconvenience.

Finally, please upload 3 photos of your home. 2 of the photos should be of your backyard or outdoor space and 1 photo of your indoor living space. You may only upload one photo per upload box. 

 

Note: These photos are intended to show the indoor and outdoor living space to evaluate if the space matches the animals' individual needs, please no photos of your other pets or family members (even if they are adorable!). Thank you!

I understand that a pet is a lifelong commitment. I am ready to make a commitment to feed, protect, shelter and care for my pet for the rest of his/her life.
